Before you assume the game is broken
A skill has a red lock
Read the words beneath the lock. A skill point only helps when the skill shows a point cost; story, challenge, and collectible requirements must be completed another way.

Open the quest menu and check the campaign heading, not only the nearest map icon. Act 2’s chains overlap and may wait for another region or fortress.

A landmark sketch is usually asking you to match shapes in the world. Compare walls, ridgelines, arches, towers, and elevation instead of waiting for a normal waypoint.
An enemy came back again
The Nemesis system can deliberately preserve history with an Orc. Check the captain’s new traits and weaknesses before repeating the same attack that failed last time.
